This form is completed entirely online. Nothing needs to be printed, stamped or delivered.

Three people sign it, in this order:

You, the employee – signature, photo and a picture of your identity document
Witness 1 – name, ID number and signature
Witness 2 – name, ID number and signature

Have both witnesses with you before you start, since they sign on this same device at the end. A witness must be an adult known to you who is not nominated to benefit in the Funeral or Gratuity Lumpsum sections.

3. NOMINATION OF GUARDIANS

I nominate the below individuals to be responsible for my minor children’s gratuity benefit in the event I pass on. If anyone nominated in sections 1 or 2 is under 21, a guardian must be named here.

When is this section required?
If anyone you named under 1. Funeral Benefit Nomination is under 21, they cannot claim the benefit themselves, so a guardian must be named here. The age is worked out from the identity number you entered. If everyone named there is 21 or older, this section is optional.

4. NOMINATION OF DEPENDENTS/BENEFICIARIES TO BENEFIT FROM DEATH GRATUITY LUMPSUM

I hereby give notice of my wish that the gratuity, which may be payable upon my death, be paid to the beneficiaries and in the proportion mentioned by me below. (Ensure the sum of percentages allocated is 100%) Members can allocate any percentage of their choice to spouses and children or other dependents.

How to fill in the four tables below.

List every person you want to benefit, across whichever of the four tables apply.
Every person you name must be given a percentage.
The percentages of everyone named, added together across all four tables, must come to exactly 100%.
Enter numbers only, for example 25 or 12.5. Do not type the % sign.
It is mandatory that all your children below 21 years are allocated.

A running total appears under the last table as you type, so you can see how much is left to allocate. The form cannot be submitted until it reaches 100%.
